Chestnut Health Company in Payson, AZ seeks a compassionate Nurse Practitioner to join our physiatry team serving residents in Skilled Nursing Facilities. This role emphasizes collaborative, patient-centered rehabilitation to restore function and independence and to improve quality of life for patients.

You will perform thorough assessments, partner with therapists and nursing staff to craft treatment plans, document progress in EMR, and educate patients and families.

Competenze

Active license to practice in the state of employment
Current Controlled Substances license (required on start)
Current DEA license (required on start)
ANCC or AANP nurse practitioner certification
Desire to work in Physiatry/PM&R

Mansioni

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs
Conduct thorough patient assessments including histories and exams to determine rehabilitation goals
Monitor patient progress and adjust treatment plans to optimize functional outcomes
Participate in multidisciplinary team meetings and educational activities
Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems
Provide education and counseling to patients and families about rehabilitation and post-discharge care

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
