Chestnut Health Company is seeking a Nurse Practitioner to join our physiatry team in a skilled nursing facility setting. Qualifikationen

• Active Nurse Practitioner certification in the required state
• DEA license and controlled substances registration required at start date
• ANCC or AANP board certification is expected
• Interest in physiatry/PM&R with SNF experience considered a plus
• Experience in SNF settings and orthopedic exposure are advantageous

• Collaborate with therapists and nurses to develop and implement comprehensive rehab plans
• Conduct patient assessments, histories, and examinations to set rehab goals
• Monitor progress and adjust treatment plans to improve function and independence
• Participate in multidisciplinary meetings and ongoing education to enhance clinical skills
• Document evaluations, plans, progress notes, and discharge summaries in EMR
• Provide patient and family education about rehabilitation processes and post-discharge care

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Requirements

Qualifications:

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
• Current DEA License (required upon start date)
• ANCC Certificate/AANP Certificate
• Desire to work in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

License/Certification:

• Certified Nurse Practitioner (Required)
