Chestnut Health Company seeks a Nurse Practitioner to join our physiatry team serving Skilled Nursing Facilities. New graduates encouraged; opportunity to impact rehabilitation outcomes in a collaborative, patient-centered environment.

You will partner with therapists and nurses to develop treatment plans, perform assessments, and help patients regain function. We offer competitive compensation, medical/dental insurance, PTO, CME allowance, malpractice coverage, 401(k), and no after-hours call.

Competenze

• Active NP license in the state of employment.
• Current CS license and DEA registration required on start.
• ANCC/AANP certification preferred.

Mansioni

• Collaborate with therapists and nurses to create rehab plans.
• Conduct comprehensive patient assessments to set rehab goals.
• Monitor progress and adjust plans to improve function.
• Participate in multidisciplinary meetings and education.
• Document evaluations, plans, progress, and discharge notes in EMR.
• Educate patients and families about rehab process and aftercare.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Requirements

Qualifications:

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
• Current DEA License (required upon start date)
• ANCC Certificate/AANP Certificate
• Desire to work in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

License/Certification:

• Certified Nurse Practitioner (Required)
