American Physiatry in New Jersey is seeking a compassionate Nurse Practitioner to join our care team in skilled nursing facilities. You will work closely with a Physiatrist, focusing on pain control, function, and rehabilitation to improve residents' quality of life.

Qualifications

  • Licensed Nurse Practitioner with a valid New Jersey certification.
  • Experience working independently while effectively managing your own caseload.
  • Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.

Responsibilities

  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
  • Educate patients and their families on treatment plans, health maintenance, and preventive care.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
