Job summary

American Physiatry is seeking a compassionate Nurse Practitioner to join our healthcare team in skilled nursing facilities. You will work closely with physical therapy to focus on pain control and functional recovery, while maintaining comprehensive patient care and documentation.

The ideal candidate is an NP licensed in North Carolina with the ability to manage a caseload independently in a fast-paced, team-oriented environment.

Qualifications

  • Experience working independently while managing your own caseload.
  • Accurate and thorough clinical documentation in a timely manner.

Responsibilities

  • Conduct thorough assessments of patients under supervision of a Physiatrist.
  • Develop and implement individualized care plans to improve function and rehabilitation.
  • Collaborate with the healthcare team to manage medications and therapy recommendations.
  • Educate patients and families on treatment plans, health maintenance, and preventive care.
  • Monitor and evaluate patient progress, adjusting plans as needed.

Key Responsibilities
  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
  • Educate patients and their families on treatment plans, health maintenance, and preventive care.

Qualifications
  • Licensed Nurse Practitioner with a valid North Carolina certification.
