Job summary

NOMS Healthcare, based in Oberlin, OH, seeks a Nurse Practitioner to deliver high-quality, patient-centered care in a collaborative practice. You will assess, diagnose, treat, and manage a broad range of conditions while partnering with physicians and interdisciplinary teams.

Qualifications

• Successful completion of an accredited Nurse Practitioner program.
• OH NP licensure and DEA registration as appropriate.
• BLS certification and Epic EMR experience preferred.

Responsibilities

• Perform comprehensive health assessments, physical exams, and medical history reviews.
• Order, interpret, and evaluate laboratory tests, diagnostic procedures, and imaging studies.
• Develop, implement, and manage treatment plans based on patient findings.
• Diagnose and manage acute and chronic conditions within scope and guidelines.
• Prescribe medications and treatments per state regulations and protocols.
• Educate patients and families regarding diagnoses, medications, and preventive care.
• Collaborate with physicians and care teams to coordinate patient care.
• Maintain accurate medical documentation in the EMR and ensure continuity.

The Challenge (Job Summary)

As a Nurse Practitioner, you'll play a vital role in delivering high-quality, patient-centered care within a collaborative practice environment. You will assess, diagnose, treat, and manage patients across a variety of clinical conditions while working in partnership with physicians and interdisciplinary healthcare teams.

This position is responsible for providing comprehensive patient care services, promoting health and wellness, educating patients and families, and supporting continuity of care. Through clinical expertise, compassion, and collaboration, you will help improve patient outcomes and advance the mission of NOMS Healthcare.

Key Responsibilities & Essential Functions:

• Perform comprehensive health assessments, physical examinations, and medical history reviews.
• Order, interpret, and evaluate laboratory tests, diagnostic procedures, and imaging studies.
• Develop, implement, and manage treatment plans based on patient assessment findings.
• Diagnose and manage acute and chronic medical conditions within scope of practice and organizational guidelines.
• Prescribe medications and treatments in accordance with state regulations and established protocols.
• Administer medications, injections, and other therapeutic interventions as appropriate.
• Perform clinical procedures consistent with education, training, licensure, and organizational policies.
• Educate patients and families regarding diagnoses, medications, treatment plans, preventive care, and wellness initiatives.
• Collaborate with physicians and interdisciplinary care teams to coordinate and manage patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Maintain accurate, timely, and complete medical documentation within the electronic medical record.
• Review patient records, test results, and consultation reports to support continuity of care.
• Respond to patient inquiries, triage patient concerns, and provide clinical consultation as appropriate.
• Recommend community resources and support services to address patient and family needs.
• Ensure compliance with HIPAA, OSHA, quality standards, clinical guidelines, and organizational policies.
• Participate in quality improvement initiatives, continuing education, and professional development activities.
• Maintain strict patient confidentiality and professional standards at all times.

• A dvanced knowledge of nursing theory, evidence-based practice, and patient care standards.
• Ability to provide compassionate, patient-centered care across diverse patient populations.
• Knowledge of healthcare regulations, safety standards, and clinical best practices.
• Strong interpersonal, communication, and relationship-building skills.
• Ability to educate patients, families, and caregivers effectively.

• Ability to adapt quickly and respond effectively in emergency or urgent situations.
• Proficiency with electronic medical records; Epic experience preferred.
• Commitment to quality improvement, patient safety, and exceptional patient experiences.

Education & Experience:

• Successful completion of an accredited Nurse Practitioner program required.
• M aster of Science in Nursing (MSN) preferred.
• Minimum of three (3) years of professional nursing experience required.
• Minimum of two (2) years of Nurse Practitioner experience preferred.
• Specialty-specific clinical experience may be preferred based on practice needs.

Licenses & Certifications:

• Current Ohio Certified Nurse Practitioner (CNP) or Advanced Practice Registered Nurse (APRN) license required.
• C urrent DEA registration, if applicable to specialty and prescribing responsibilities.
• Current Basic Life Support (BLS) certification required.
• Additional specialty certifications may be required based on practice area.

The ideal candidate must be able to view computer screens for long durations and work effectively in a fast-paced, collaborative office setting. For patient care–related roles, the position may also require standing for long periods and the ability to lift or assist patients, medical equipment, or supplies — generally up to 25–50 pounds, as needed to support safe and effective care.
