Job summary

One to One Health is seeking a Board Certified Nurse Practitioner or Certified Physician Assistant to join our Normal, IL clinic. This role involves direct and indirect patient care under a supervising physician, including wellness physicals, acute and chronic care management, and health promotion activities.

You will collaborate with a multidisciplinary team, document in EMR, and participate in ongoing quality improvement while ensuring high ethical standards and compassionate service.

Qualifications

  • Board Certified Nurse Practitioner or Certified Physician Assistant with an active license.
  • DEA license preferred and BLS certification.
  • 1 year experience in family medicine, internal medicine or urgent care setting.

Responsibilities

  • Perform pre-employment and annual wellness physicals.
  • Evaluate and treat health problems using evidence-based protocols under supervising physician.
  • Provide health screenings and disease prevention programs.
  • Evaluate and treat patients for injury and illness.
  • Enter patient data into the EMR system accurately.

One to One Health is looking for a talented and compassionate Provider to join our team in Normal, IL! (Relocation assistance available to eligible applicants)

This is an exciting opportunity to provide direct and indirect patient care. You will be an integral part of the clinic's success. You will be responsible for evaluating and treating, managing acute health problems using evidence-based protocols, and creating and implementing appropriate treatment plans under the supervising physician. The position promises a wide range of challenges and opportunities, with the chance to significantly contribute to health and safety.

ESSENTIAL FUNCTIONS
  • Perform pre-employment and annual wellness physicals
  • Manage acute and chronic health problems using evidence-based protocols creating and implementing appropriate treatment plans under supervising physician’s protocols
  • Provide health screenings and other disease prevention programs
  • Evaluation and treatment of patients for injury and illness
  • Report and review cases with the medical director and client managers
  • Oversee respirator and auditory testing for employees by clinic staff
  • Provide age-appropriate assessments and interventions to meet the needs of teammates.
  • Uphold high ethical standards and practice within the scope of practice outlined in the written protocol
  • Provide patient education, health coaching, and patient health advocacy
  • Order and analyze diagnostic tests
  • Monitors the effectiveness of interventions
  • Collaborates with multidisciplinary team members by making appropriate referrals
  • Enter patient data into the EMR system accurately and promptly
  • Assist in all areas of the clinic, if needed
  • Perform related duties as required
REQUIRED SKILLS/ KNOWLEDGE

QUALIFICATIONS/EXPERIENCE
  • Board Certified Nurse Practitioner or Certified Physician Assistant with an active license, 1 year experience in family medicine, internal medicine or urgent care setting required.
