Job summary

The CARIN Alliance is seeking a Nurse Practitioner to deliver direct patient care across medical centers, nursing homes, and home health settings in Virginia's Norfolk area.

Responsibilities include geriatric assessments, physical exams, diagnosis, treatment planning, health education, referrals, and documentation aligned with quality standards. Collaboration with physicians and a healthcare team is essential, with independent practice within a professional agreement.

Qualifications

  • Current RN licensure and NP certification in the practicing state.
  • BLS certification; Board certification preferred.
  • Minimum of 2 years’ clinical experience in Family Medicine, Internal Medicine, or Geriatrics.
  • Strong critical thinking, communication, and interpersonal skills.
  • Detail-oriented with excellent time management and computer skills.
  • Willingness to travel locally as needed.

  • Order and interpret diagnostic tests, prescribe medications, and refer patients to specialists as needed.
  • Participate in clinical rounds, documentation, and multidisciplinary team collaboration.
  • Recognize urgent situations, initiate life-saving procedures, and communicate effectively with patients and families.
  • Support discharge planning and transition to lower levels of care, ensuring patient-centered approaches.

Job description

Role Overview

The Nurse Practitioner (NP) provides direct patient care across various settings, including medical centers, nursing homes, and home health. Responsibilities encompass geriatric assessments, physical exams, diagnosis, treatment, care planning, health education, referrals, follow-up, and documentation following quality standards. The NP collaborates with physicians and healthcare team members, functions independently within a practice agreement, and may perform invasive procedures upon competency approval.
