Qualifications

Job Summary
Under the direction of the Supervising Physician, the Primary Care Nurse Practitioner (NP) provides comprehensive primary care services to patients. This role is responsible for assessing, diagnosing, treating, and managing acute and chronic medical conditions while promoting preventive care and overall wellness.

The NP works collaboratively with physicians, care coordinators, medical assistants, and other healthcare team members to deliver high-quality, patient-centered care. The primary objective of this position is to improve patient outcomes, reduce hospitalizations, and support patients in managing their health effectively.

Duties, Functions, and Responsibilities

  • Perform comprehensive physical assessments and review patient medical histories.
  • Order, interpret, and evaluate diagnostic tests, including laboratory work and imaging studies.
  • Diagnose and treat a wide range of acute, chronic, and infectious health conditions.
  • Prescribe medications and develop appropriate treatment plans in accordance with scope of practice and applicable regulations.
  • Educate patients and families regarding diagnoses, treatment plans, medication usage, and preventive health measures.
  • Maintain accurate, timely, and complete documentation in the electronic medical record (EMR) system.
  • Collaborate with physicians, medical assistants, care coordinators, and administrative staff to ensure continuity and coordination of care.
  • Promote preventive care and healthy lifestyle practices among patients.
  • Participate in clinical meetings, case reviews, and ongoing professional development activities.
  • Comply with all clinical protocols, organizational policies, state regulations, and applicable laws governing Nurse Practitioner / Physician Assistant practice.
