Job summary

Housing Works seeks a Nurse Practitioner to provide high-quality, patient-centered care in collaboration with physicians and healthcare professionals. You will diagnose and manage acute and chronic conditions, promote preventive care, and build strong patient relationships.

The role requires NYS NP licensure, NYS RN license, 2–4 years NP experience, and experience with HIV/AIDS primary care. Bilingual EN/ES is a plus; EHR experience preferred.

Qualifications

  • Licensed NP in NYS with current registration.
  • RN license in NYS.
  • 2–4 years NP experience in clinical settings.
  • Experience with HIV/AIDS primary care preferred.
  • Experience with substance use management preferred.
  • Familiarity with EHR systems preferred.

Responsibilities

  • Assess and interpret medical histories; perform exams; develop treatment plans.
  • Refer to collaborating physician when indicated.
  • Order lab tests and interpret results; report to Health Department as required.
  • Prescribe medications within protocols.
  • Educate patients and provide counseling on health options.

Job description

Overview:The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to provide high-quality, patient-centered care. This role involves diagnosing and managing a wide range of acute and chronic conditions, promoting preventive care, and fostering strong patient relationships.

Salary Range:$140,000-$150,000 annually commensurate with experience

Responsibilities
  • Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate; identifies problems; formulates a plan of treatment; provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed.
  • Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral course when indicated.
  • Performs or orders laboratory tests and interprets the results as part of the health management. Reports communicable disease in a timely manner to the appropriate Health Department as per health center protocol.
  • Orders medications.
  • Follows established treatment guidelines as indicated by Health Center protocols.
  • Conducts health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options.
  • Completes the client’s clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center
  • Understands the medical emergency procedures and is familiar with the location of the emergency kit/supplies in all clinic settings.
  • Participates in quality improvement/assurance activities.
  • Participates in continuing medical education activities to enhance professionalism and to ensure quality of care.
  • Provides on-call coverage as scheduled.
  • Works collaboratively with all members of the health center to build a supportive team.
  • Applies infection control measures according to Health Center Policies and Procedures.
  • Maintains professionalism and adheres to client confidentiality.
Job Requirements
  • Licensed to practice as a NYS Nurse Practitioner.
  • Licensed and currently registered as a Registered Nurse in the State of New York.
  • 2 to 4 years of experience as a Nurse practitioner
  • Experience and knowledge of HIV/AIDS primary care preferred.
  • Experience and knowledge of managing clients with substance abuse preferred.
  • Previous experience with an electronic health record, preferred
  • Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated
  • Auditory acuity to handle phone calls and interact extensively with patients
  • Maintains CMEs/CEUs required for licensure
  • Maintains CPR certifications
  • Maintains Infection Control certification.
  • Maintains DEA registration.
  • Certified by ANCC or AANP
  • Bilingual English/Spanish preferred, but not required
  • Preference for individuals with experiencing working in a community-based setting, particularly with patients who are LGTBQIA+, and/or have experienced homelessness, substance use, and/or mental health challenges
