Job summary

Job description

Overview

The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to provide high-quality, patient-centered care. This role involves diagnosing and managing a wide range of acute and chronic conditions, promoting preventive care, and fostering strong patient relationships.

Salary Range

$140,000‑$150,000 annually commensurate with experience

Responsibilities
  • Assess and interpret medical and psycho-social histories; perform physical examinations (including gynecological and breast examination as appropriate); identify problems; formulate a plan of treatment; provide education and referral as indicated; evaluate the effectiveness of the interventions prescribed.
  • Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to the collaborating physician or other appropriate referral course when indicated.
  • Perform or order laboratory tests and interpret the results as part of the health management; report communicable disease in a timely manner to the appropriate Health Department as per health center protocol.
  • Order medications.
  • Follow established treatment guidelines as indicated by Health Center protocols.
  • Conduct health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options.
  • Complete the client’s clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center.
  • Understand the medical emergency procedures and be familiar with the location of the emergency kit/supplies in all clinic settings.
  • Participate in quality improvement/assurance activities.
  • Participate in continuing medical education activities to enhance professionalism and ensure quality of care.
  • Provide on-call coverage as scheduled.
  • Work collaboratively with all members of the health center to build a supportive team.
  • Apply infection control measures according to Health Center Policies and Procedures.
  • Maintain professionalism and adhere to client confidentiality.
Job Requirements
  • Licensed to practice as a NYS Nurse Practitioner.
  • Licensed and currently registered as a Registered Nurse in the State of New York.
  • 2 to 4 years of experience as a Nurse Practitioner.
  • Experience and knowledge of HIV/AIDS primary care preferred.
  • Experience and knowledge of managing clients with substance abuse preferred.
  • Previous experience with an electronic health record preferred.
  • Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated.
  • Auditory acuity to handle phone calls and interact extensively with patients.
  • Maintain CMEs/CEUs required for licensure.
  • Maintain CPR certifications.
  • Maintain Infection Control certification.
  • Maintain DEA registration.
  • Certified by ANCC or AANP.
  • Bilingual English/Spanish preferred, but not required.
  • Preference for individuals with experience working in a community-based setting, particularly with patients who are LGBTQIA+ and/or have experienced homelessness, substance use, and/or mental health challenges.
