Job summary

MJHS Medical Associates, P.C. seeks a qualified Nurse Practitioner to provide and coordinate primary care for Advantage members in participating long term care facilities and home settings.

In collaboration with physicians, you will develop care plans, ensure preventive care access, and serve as a clinical resource to providers and facilities, supporting member-centered care and disease management across geriatric populations.

Qualifications

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required.
• One to two years clinical experience as a nurse practitioner.
• Minimum one year in geriatric specialty.
• Previous RN experience in acute, subacute and/or long term care setting preferred.
• Prior experience in managed care a plus.
• NYS RN/NP License, Registration, DEA certificates.

Responsibilities

• Provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities.
• Develop care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives.
• Provide and coordinate primary clinical care services for assigned Nursing Home Residents.
• Act as a clinical resource to the participating providers and facilities.
• Act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
