Job summary

Senior Community Care of Colorado PACE seeks a Nurse Practitioner to deliver primary care to PACE participants, conducting physical assessments and developing care plans in collaboration with an Interdisciplinary Team. The role involves in-person evaluations, chronic disease management, and both clinic and home-based care, supporting frail elders.

Qualifications

  • Graduate of a school of professional nursing required.
  • Licensed as Registered Nurse (RN) in the state of employment.
  • Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
  • Prescriptive authority required within one year of hire.
  • Certified as a Geriatric Nurse Practitioner (GNP) preferred.
  • Experience: minimum of one year with frail elderly population.
  • Two years as NP in geriatric setting preferred.

Responsibilities

  • Under the supervision of the Medical Director provides primary care to participants.
  • Performs physical assessments of new PACE enrollees and semi-annual reassessments; develops and implements plans of care.
  • Evaluates participant conditions and provides appropriate treatment.
  • Provides education to participants and caregivers.
  • Works as a member of the Interdisciplinary Team (IDT).
  • Assesses and cares for frail elder participants according to Senior CommUnity Care standards.
  • Performs in-person history and physical assessments of new PACE participants.
  • Coordinates with IDT to develop comprehensive plans of care.
  • Manages chronic diseases and evaluates acute illnesses in clinic or home settings.
  • Oversees care for participants in Skilled Nursing Facilities and participates in on-call rotation.

Hourly Range: $126k - $140k annually, based on experience.

Schedule: Monday-Friday 8am-5pm; occasional weekend for on-call hours *NPs are required to participate in the on-call rotation.

This position will work at both sites (Eckert & Montrose), sharing time between each.

What You Bring (Requirements)
  • Education: Graduate of a school of professional nursing required.
  • Licensed as Registered Nurse (R.N.) in the state of employment.
  • Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
  • Prescriptive authority required within one year of hire.
  • Certified as a Geriatric Nurse Practitioner (G.N.P.) preferred.
  • Experience: A minimum of one year of experience in working with the frail and elderly population required.
  • Two (2) years of experience as a nurse practitioner in a geriatric setting preferred.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Skills and Knowledge: Working knowledge of the PACE regulations desired.
What You Bring (Requirements)
  • Under the supervision of the Medical Director provides primary care to participants.
  • Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care to Senior CommUnity Care program participants.
  • Evaluates participant physical complaints and provides appropriate treatment.
  • Provides participants and caregiver teaching and education.
  • Functions as a member of the Interdisciplinary Team (IDT).
  • Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Senior CommUnity Care standards.
  • Performs in person comprehensive history and physical assessments of new Senior CommUnity Care PACE participants.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person re-assessment semi-annually and as needed.
  • Manages, evaluates and treats participants with chronic diseases, maximizing interventions in the PACE clinic or the participant's home prior to initiating a higher level of care.

  • Responsible for the oversight of the care of PACE participants who reside in a Skilled Nursing Facility.
  • Participates in the on-call rotation.
  • Performs other duties as required or requested.
