Certified-NP for Home Health Assessments

Urgent Need: As soon as credentialed or by October 15 - end of year

Schedule: 8 hour days, 5 days a week (M-F or W-F example)

Hours of operation 8 AM - 8 PM (8 hour days only, this includes chart and travel time)

Base pay range

$75.00/hr - $85.00/hr

Scope of Practice / Responsibility

Visit Type A. Level 2–3 Diagnostic Visits — close quality gaps for health plans:

• Full annual wellness + labs/diagnostics
• Assess Social Determinants of Health; document needs/recommend resources
• Care Guides convert signed charts into a 30-day community resource for the member

Visit Type C. Telehealth Annual Wellness— Limited
