Optum Home & Community Care, part of the Optum family, is creating a new integrated care model that responds to patients’ physical, mental, and social needs. As a member of our Senior Community Care (SCC) team, you will provide care to patients in nursing homes, senior housing, and assisted living settings, delivering comprehensive assessments, documenting findings, and participating in care coordination.
The Patient Connect program is a one‑time comprehensive visit for patients who have recently been hospitalized and are in a skilled rehabilitation facility. The purpose is to ensure a smooth transition from rehab to home and to prevent readmission. In this role you will capture STAR measures, educate the patient, and document all acute and chronic diagnoses. You will communicate findings to attending physicians and facility staff but will not be responsible for orders, treatments, or follow‑up. You will work independently and exercise the autonomy accorded to this position.
Compensation for this specialty generally ranges from $90 to $155 per hour. Total cash compensation includes base pay and bonus and is based on several factors, including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role.
