We are seeking a compassionate and motivated Nurse Practitioner to join our dynamic primary care team. The ideal candidate will be responsible for providing high-quality comprehensive clinical care to patients of all ages within our membership-based practice model. This includes conducting wellness exams, managing acute and chronic conditions, delivering telehealth/virtual care, providing patient education, and collaborating with our multidisciplinary team to ensure optimal outcomes. Key Responsibilities:

• Provide urgent care treatments for common illnesses and minor injuries (e.g., cold/flu, sinus infections, musculoskeletal injuries, minor lacerations).
• Deliver preventative care, wellness exams, routine health screenings, and disease prevention planning.
• Develop, implement, and monitor individualized treatment plans in collaboration with patients and supervising physicians.
• Prescribe medications and manage pharmacotherapy.
• Provide clear, thorough patient education regarding diagnoses, treatment options, lifestyle modifications, and health maintenance.
• Utilize virtual tools (phone, text, video chat) through secure, HIPAA-compliant platforms to provide timely telehealth care to practice members.
• Maintain accurate, streamlined, and timely electronic health records (EHR).
• Collaborate effectively with supervising physicians, medical assistants, and administrative staff to ensure a seamless patient experience.
