Job summary

NewCare MD in Mississippi is seeking a compassionate Nurse Practitioner to join our membership-based primary care team. In our Direct Primary Care model, you’ll deliver high-quality clinical care with extended visit times, focusing on transparency, trust, and time with patients.

Responsibilities include wellness exams, managing acute and chronic conditions, telehealth care, patient education, EHR documentation, and collaboration with a multidisciplinary team to deliver outcomes.

Responsibilities

• Conduct comprehensive health assessments and order/interpret diagnostics.
• Diagnose and manage acute and chronic primary care conditions.
• Provide urgent care for common illnesses and minor injuries.
• Deliver preventative care and routine screenings.
• Develop and monitor individualized treatment plans with supervising physicians.
• Prescribe medications and manage pharmacotherapy.
• Educate patients about diagnoses, options, lifestyle changes.
• Use HIPAA-compliant telehealth tools for care.
• Maintain accurate EHR and collaborate with staff.
• Stay current with evidence-based primary care practices.
