Job summary

All Saints Hospice in Lake Charles, LA is seeking a Nurse Practitioner to provide hospice care, evaluate, diagnose, develop plans, and prescribe treatment under physician supervision. You will perform histories and physicals, assess eligibility, and contribute to the care team to meet medical and emotional needs.

Qualified candidates have an NP degree from an accredited program, current state license, CPR, and a DEA license if required.

Qualifications

  • Graduate from an accredited School of Nursing and NP program.
  • Current DEA license if required by state regulations.
  • Current nursing license in state of practice.
  • Master's degree with NP clinical experience.
  • One year hospice or palliative care experience preferred.
  • Certification from an approved certifying body for APN as required by state.
  • CHPN or Geriatrics certification preferred.

Responsibilities

  • Provide hospice medical care, palliate and manage terminal illness under physician supervision.
  • Perform history and physical examinations evaluating medical condition and history.
  • Assess hospice eligibility prior to benefit periods and recommend cert/recertification.
  • Maintain accurate clinical notes and communicate with physician and care team.
  • Ensure compliance with state and federal regulations.
  • Participate in interdisciplinary meetings to meet patient and family needs.
  • Perform additional responsibilities as assigned.

Responsibilities
  • Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
  • Completing history and physical examinations by evaluating the patient's medical condition and health history
  • Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Completing and maintaining accurate clinical notes
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
  • Ensuring compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned
Qualifications
  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
  • Current DEA license, if applicable to state regulations
  • Current nursing license in state of practice
  • Current CPR certification
  • Master's degree with minimum of one year NP experience
  • One year hospice or palliative care experience preferred
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
  • Commitment to clinical and documentation excellence
