Job summary

All Saints Hospice in Louisiana is seeking a Nurse Practitioner to lead hospice medical care, working under a collaborating physician to deliver compassionate end-of-life services.

The role focuses on evaluations, care planning, prescribing medications, and coordinating with a multidisciplinary team to meet patients' medical and emotional needs. APRN with hospice experience preferred; DEA license may be required by state regulations.

Qualifications

• Must graduate from an accredited School of Nursing and accredited NP program.
• Current nursing license in state of practice and CPR certification.
• Current DEA license, if applicable to state regulations.
• Master’s degree with NP certification; hospice experience preferred.

Responsibilities

• Provide medical care and palliation for terminal illness under physician supervision.
• Perform history and physical exams evaluating patient condition.
• Assess hospice eligibility prior to benefit periods with face-to-face visits.
• Recommend eligibility for certification or recertification.
• Maintain accurate clinical notes.
• Coordinate with physician and interdisciplinary team for patient and family needs.
• Ensure compliance with all state and federal regulations.
• Additional duties as assigned.

Responsibilities

• Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
• Completing history and physical examinations by evaluating the patient's medical condition and health history
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Completing and maintaining accurate clinical notes
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
• Ensuring compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Qualifications

• Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
• Current DEA license, if applicable to state regulations
• Current nursing license in state of practice
• Current CPR certification
• Master’s degree with minimum of one year NP experience
• One year hospice or palliative care experience preferred
• Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
• Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
• Commitment to clinical and documentation excellence
