Job summary

Haven Hospice in Florida is seeking a Nurse Practitioner to provide comprehensive hospice medical care, including evaluation, symptom management and treatment planning.

You will perform history and physicals, participate in interdisciplinary teams, and ensure regulatory compliance while maintaining thorough clinical documentation. A current NP license, DEA where required, and hospice experience are preferred.

Qualifications

• Master's degree required with NP clinical experience.
• Licensed as a Nurse Practitioner with an active state license.
• DEA license if applicable to state regulations.
• Accreditation by an approved certifying body for advanced practice nursing.
• CHPN or geriatrics certification preferred.
• Hospice or palliative care experience preferred.

Responsibilities

• Providing medical care, palliation and management of terminal illness under supervision of a collaborating physician.
• Completing history and physical examinations evaluating medical condition and health history.
• Completing hospice face-to-face visit requirements to assess eligibility.
• Recommending patient eligibility/certification for hospice care to medical director.
• Maintaining accurate clinical notes.
• Communicating with physician and participating in interdisciplinary meetings.
• Ensuring compliance with state and federal requirements.
• Additional responsibilities as assigned.

Education

Master's degree in Nursing

Graduate from accredited School of Nursing and NP program

Current nursing license in state of practice

Current DEA license

Responsibilities

• Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
• Completing history and physical examinations by evaluating the patient's medical condition and health history
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Completing and maintaining accurate clinical notes
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
• Ensuring compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Qualifications

• Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
• Current DEA license, if applicable to state regulations
• Current nursing license in state of practice
• Current CPR certification
• Master's degree with minimum of one year NP experience
• One year hospice or palliative care experience preferred
• Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
• Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
• Commitment to clinical and documentation excellence
