Job summary

Humana, CenterWell Senior Primary Care in Indianapolis, seeks a Nurse Practitioner to deliver comprehensive outpatient care for adults and seniors in a on-site setting. You will collaborate with an interdisciplinary team, maintain records, and exercise independent clinical judgment.

The role requires a Master’s in nursing with board certification, an active NP license, and 2+ years of NP experience. On-call coverage by phone is required quarterly as part of program participation.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.

Required Qualification

Preferred Qualification

• Specialty training in Primary Care, Family Medicine, Internal Medicine, or Geriatrics.
• Experience working with senior populations or in Value-Based Care settings.
• Proficiency with electronic health records Athena Health and digital documentation.

Work Environment

Outpatient on-site clinical setting.

Standard schedule: Monday-Friday, 8:00 AM - 5:00 PM.

TB Statement

This role is considered patient facing and is part of Humana\'s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
