Job summary

Humana CenterWell Senior Primary Care in Louisville, KY seeks a PRN Primary Care Nurse Practitioner to join a patient-centered, collaborative team. You will work onsite delivering outpatient care to adults and seniors, with emphasis on comprehensive records, referrals, and coordinated care.

Requirements include a Master's in nursing, board-certified NP, active unrestricted license, and at least two years of NP experience.

Qualifications

• Master's degree in nursing with board certification.
• Active, unrestricted NP license in the state of practice.
• Minimum two years' Nurse Practitioner experience.
• EMR experience and adapts well to new technologies.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement programs.

Responsibilities

• You will report to the Associate Medical Director: Dr. Tamea Evans .
• You will work fully onsite in Louisville, KY.
• Deliver comprehensive outpatient care to adult and senior patients.

Required Qualification

• Master's degree in nursing with board certification.
• Active, unrestricted NP license in the state of practice.
• Minimum two years' Nurse Practitioner experience.
• EMR experience and adapts well to new technologies.

Preferred Qualification

• Specialty training in Primary Care, Family Medicine, Internal Medicine, or Geriatrics.
• Experience working with senior populations or in Value-Based Care settings.
• Proficiency with electronic health records Athena Health and digital documentation.

Work Environment

• Outpatient on-site clinical setting.
• Standard schedule: Monday-Friday, 8:00 AM - 5:00 PM.
• TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

$149,800 - $206,100 per year

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
