Job summary

Carease Health is seeking a dedicated Nurse Practitioner to join our team in a hybrid role serving residents in skilled nursing facilities. You will provide preventive and geriatric care, balancing on-site visits with remote work supported by a scribe team for efficient documentation.

Collaborate with clinical directors and facility staff to ensure patient-centered care, participate in quality improvement initiatives, and maintain strong communication within the multidisciplinary team.

Qualifications

• 1+ year of experience in Geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.
