• Collaborate with healthcare providers and facility staff to develop and implement individualized care plans.
• Participate in the Chronic Care Management (CCM) and Collaborative Care Management (CoCM) programs to address mental health and well-being.
• Stay up-to-date with industry best practices and maintain accurate and timely documentation.
• Build strong relationships with facility staff, patients, and families to ensure patient-centered care.
