Job summary

Carease Health in the United States is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team. The role delivers preventive care to residents in skilled nursing facilities, combining on-site visits with remote work from home in a hybrid arrangement.

You will work with a team to monitor vital signs, develop care plans, document promptly with a dedicated scribe, and participate in multidisciplinary meetings.

Qualifications

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills

Responsibilities

• Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs

• Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team
• Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine
• Build strong relationships with facility staff, patients, and families, while maintaining a patient-centered approach
• Stay current with industry developments and best practices through ongoing education and training
