Chestnut Health is seeking a compassionate Nurse Practitioner to join our Physiatry team in Illinois. This role focuses on rehabilitation and restoring function, with collaborative care across therapists, nurses, and physicians.

Qualifications

• Active Illinois NP license or eligibility
• ANCC or AANP certification
• Interest in Physiatry/PM&R
• Eligible to work in the United States
• Current Controlled Substance License preferred
• Current DEA License preferred
• Orthopedic experience (nice to have)
• Physiatry experience (nice to have)
• Skilled nursing experience (nice to have)

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans.
• Conduct thorough patient assessments, including histories, exams, and diagnostics to set rehab goals.
• Monitor progress and adjust plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and education sessions.
• Maintain accurate documentation in electronic medical records including evaluations, plans, progress and discharge notes.
• Educate patients and families about rehab process, outcomes, and post-discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

• Actively licensed as a nurse practitioner in Illinois or hold the certification required to practice in the state of employment
• ANCC or AANP certification
• Interest in working in Physiatry/Physical Medicine and Rehabilitation
• Eligible to work in the United States
• Current Controlled Substance License preferred
