Job summary

The Judge Group is actively seeking an Advanced Practice Clinician (NP/PA) to support the ISNP Program in Colton, CA. This full-time position offers 40 hours per week, Monday–Friday, 8:00 AM–5:00 PM, with no on-call, no weekends, and no holidays, enabling a true work-life balance.

You will join an interdisciplinary team delivering longitudinal, coordinated clinical services to high-risk members in facility-based settings, emphasizing relationship-based care and meaningful patient outcomes.

Qualifications

• Active, unrestricted NP or PA license in your state of residence (or ability to obtain by start date)
• NP: Graduate of an accredited MSN or DNP program
• PA: Graduate of an accredited Physician Assistant program
• Active national board certification (NP: AANP or ANCC; PA: NCCPA)
• Current DEA licensure/prescriptive authority
• Ability to meet physical job requirements
• Collaboration with care team and ability to obtain a collaborative practice agreement if required by state
• Full COVID-19 vaccination per requirements

About The Role

The Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) will support the Senior Community Care (SCC) program, a longitudinal, integrated care model designed to coordinate and deliver clinical services to high‑risk members in facility-based settings. This role is part of an interdisciplinary care team focused on improving outcomes for complex, vulnerable populations.

This position offers true work–life balance with no on‑call, no weekends, and no holidays. Providers enjoy flexible scheduling and the opportunity to deliver meaningful, relationship‑based care.

Primary Responsibilities

• Comprehensive Assessments — Perform full clinical evaluations for ISNP members


• Acute & Chronic Condition Management — Provide ongoing medical and behavioral health management

Required Qualifications

• Reliable transportation


• Ability to obtain a collaborative practice agreement (if required by state)


• Full COVID‑19 vaccination (per federal, state, client requirements)


• Ability to pass pre‑employment drug screening

Preferred Qualifications

• 1+ year of clinical practice experience (long‑term care preferred)


• Experience caring for patients with complex medical, behavioral, or social needs


• Knowledge of geriatrics, chronic illness, and advanced illness/end‑of‑life care


• Proficiency with electronic medical records and digital documentation
