The Judge Group is seeking an experienced Advanced Practice Clinician (NP or PA) to join our Senior Community Care program (ISNP) in Richland County, SC. The role offers a Monday-Friday 8 AM-5 PM schedule with true work-life balance, no on-call, no weekends, and no holiday requirements.

You will perform comprehensive assessments and manage complex medical and behavioral conditions as part of an interdisciplinary care team.

Qualifications

Advanced Practice Clinician (Nurse Practitioner / Physician Assistant) - The Judge Group - Senior Community Care Program (ISNP)

Schedule: 40 hours/week

Shift: Monday-Friday, 8 AM-5 PM

About The Role

The Judge Group is seeking an experienced Advanced Practice Clinician (NP or PA) to join our Senior Community Care (SCC) program within the ISNP model. This integrated, longitudinal care program supports high-risk members residing in facility settings. As part of an interdisciplinary care team, you will deliver high-quality clinical care, manage complex medical needs, and support patient well-being.

This position offers true work-life balance with flexible scheduling, no on-call, no weekends, and no holiday requirements.

Key Responsibilities

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Document accurately, including ICD-10 coding
• Collaborate with interdisciplinary care teams
• Lead advanced illness and advanced care planning discussions
• Provide education and counseling to patients and caregivers

Required Qualifications

• Active, unrestricted NP or PA license in your state (or ability to obtain by start date)
• NP Education: MSN or DNP from an accredited program
• PA Education: Graduate of an accredited PA program
• Active national board certification:
  • NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
  • PA: NCCPA
• Active DEA license / prescriptive authority
• Ability to meet physical job requirements (lifting 30-lb bag, navigating stairs, kneeling for assessments)
• Reliable transportation
• Ability to obtain collaborative practice agreement (if required by state)
• Full COVID-19 vaccination (per client and regulatory requirements)
• Must pass pre-employment drug screening

Preferred Qualifications

• 1+ year of clinical experience (long-term care preferred)
• Experience supporting patients with complex behavioral, social, or functional needs
• Strong understanding of geriatrics, chronic illness, and end-of-life care
• Proficiency with EMR systems and electronic documentation
