Qualifications

• Active NP license in the state of practice.
• At least 1 year of experience in geriatric care or internal medicine.

Responsibilities

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team at Carease Health. As a leading provider of preventative healthcare, we prioritize proactive measures for enhanced physical and mental health, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive program.

About the Role:

In this hybrid position, you will deliver high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient-centered approach, and collaborate with our clinical team to provide exceptional care.

Responsibilities:

• Provide primary and preventive care to residents in skilled nursing facilities

• Conduct on-site visits and remote monitoring using RPM and CCM technologies

• Collaborate with our clinical team to develop and implement personalized care plans

• Maintain accurate and timely documentation using our innovative documentation system

• Build strong relationships with facility staff and residents to ensure seamless care transitions

• Participate in ongoing training and professional development to stay up-to-date on best practices and industry advancements

Requirements:

• Active NP license (in the state of practice)

• 1+ year of experience in Geriatric care or Internal Medicine

• Excellent communication, collaboration, and teamwork skills

• Ability to work independently and as part of a team
