About the Role:

Carease Health is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team and deliver high-quality preventative care to residents in skilled nursing facilities. As a key member of our hybrid team, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide comprehensive preventative care to residents in skilled nursing facilities, utilizing Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) strategies
• Build strong relationships with facility staff and patients, maintaining a patient-centered approach and collaborative teamwork
• Document patient care efficiently, utilizing a dedicated scribe team for support
• Participate in ongoing education and training to stay current with industry developments and best practices

Requirements:

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
• Strong patient-centered approach and ability to work effectively in a fast-paced environment
