Qualifications

Active license to practice in the state of employment.
Current Controlled Substance License required on start date.
Current DEA license required on start date.
ANCC or AANP certification.
Interest in Physiatry/PM&R.
Orthopedic rehab experience is a plus.
Physiatry experience is a plus.
Skilled nursing experience is a plus.

Responsibilities

Collaborate with therapists and nurses to develop rehab plans for diverse patients.
Conduct patient assessments, histories, and exams to set rehab goals.
Monitor progress and adjust plans to improve function and independence.
Attend multidisciplinary meetings to share knowledge and skills.
Document evaluations, plans, progress notes, and discharges in EMR.
Educate patients and families about the rehab process and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
