Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner to provide compassionate, patient-centered care for seriously ill patients in home settings. You will assess, manage symptoms, and align treatments with patients' goals while collaborating with a multidisciplinary team to deliver high-quality hospice and palliative care, including education for patients and families.

Qualifications

• Heart for serving patients and families with compassionate care.
• Nursing credential at certificate/diploma/associate/BSN preferred.
• 2 years of chronic disease management/long-term care experience; hospice background preferred.
• Reliable transportation; ability to lift 80-100 lbs.

Responsibilities

• Care for seriously ill patients to alleviate symptoms and align treatments with goals.
• Collaborate with a team of clinicians to deliver hospice and palliative care at patients' homes.
• Provide hospice service and education as part of the patient care plan.

As a hospice and palliative nurse practitioner, you will care for seriously ill patients to help alleviate burdensome symptoms and align medical treatments with patients' goals and values. You will collaborate with a team of like-minded clinicians and teammates to deliver the best care possible for your patients wherever they call home. As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
