Job summary

ACG Hospice seeks a Nurse Practitioner to provide hospice and palliative care to seriously ill patients, aligning treatments with patient goals and values. The NP will collaborate with a multidisciplinary team to deliver high-quality care for patients in their homes.

Qualifications

• 2 years experience within the last 5 years in chronic disease management/long-term care (Hospice background preferred).
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult.

As a hospice and palliative nurse practitioner, you will care for seriously ill patients to help alleviate burdensome symptoms and align medical treatments with patients\' goals and values. You will collaborate with a team of like-minded clinicians and teammates to deliver the best care possible for your patients wherever they call home. As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education.
