Job summary

Agape Care Group's ACG Hospice is seeking a nurse practitioner to provide hospice and palliative care for seriously ill patients in the community. You will partner with an interdisciplinary team to relieve symptoms and honor patient goals.

In this role you will educate families, coordinate care at home, and participate in ongoing professional development with supportive leadership. We value compassion, integrity and patient-centered service.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• Experience: Hospice background preferred; 2 years chronic disease management/long-term care within last 5 years.
• Reliable transportation and ability to assist patients with mobility and lifting needs.

Responsibilities

• Provide hospice and palliative care to seriously ill patients at home or in the community.
• Collaborate with an interdisciplinary team to align treatments with patient goals and values.
• Educate patients and families about care plans and available resources.

As a hospice and palliative nurse practitioner, you will care for seriously ill patients to help alleviate burdensome symptoms and align medical treatments with patients’ goals and values. You will collaborate with a team of like-minded clinicians and teammates to deliver the best care possible for your patients wherever they call home. As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education.

• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
