Visionary Healthcare Partners in Fairfield, CT is seeking a Nurse Practitioner to provide comprehensive, relationship-based primary care for Medicare patients across clinic, dialysis center, and home settings. This role emphasizes value-based care, chronic disease management, and collaboration with an integrated care team.

You'll spend time with patients, document encounters, and coordinate with nephrologists and other specialists.

Qualifications

• Board Certification (AANP or ANCC) required.
• Current DEA license or ability to obtain.
• Current BLS/CPR certification.
• Minimum two years NP experience in one or more of: Primary Care, Family Medicine, Internal Medicine, Nephrology, Geriatrics, Palliative Care.
• Experience managing patients with chronic diseases; working knowledge of HCC coding and value-based care documentation.

Responsibilities

• Manage a panel of medically complex Medicare patients across clinic, dialysis center, and home settings.
• Perform comprehensive health assessments and annual risk assessments.
• Develop individualized care plans to improve health outcomes.
• Educate patients and families about diagnoses, medications, and disease management.
• Coordinate care with nephrologists, PCPs, specialists, caregivers, and community resources.
• Manage chronic conditions, medications, and post-discharge follow-up.
• Help prevent unnecessary ED visits and hospital readmissions.
• Document clinical encounters and HCC coding accurately.
• Order and interpret diagnostic tests; practice independently with an integrated team.
• Maintain HIPAA compliance and regulatory requirements.

What You'll Do

You'll manage a panel of medically complex Medicare patients by delivering comprehensive, patient-centered care across clinic, dialysis center, and home settings.

Responsibilities include:

• Provide longitudinal primary care for patients with CKD and ESKD
  
• Perform comprehensive health assessments and annual health risk assessments
  
• Develop individualized care plans that support improved health outcomes
  
• Educate patients and families about diagnoses, medications, and disease management
  
• Coordinate care with nephrologists, primary care providers, specialists, caregivers, and community resources
  
• Manage chronic conditions, medication regimens, and post-discharge follow-up
  
• Help prevent unnecessary emergency department visits and hospital readmissions
  
• Accurately document clinical encounters and HCC coding
  
• Order and interpret diagnostic tests as appropriate
  
• Practice independently while collaborating closely with physicians and an integrated care team
  
• Maintain compliance with HIPAA and all regulatory requirements
  

This position offers a blend of:

Requirements

What We're Looking For

Required

• Board Certification (AANP or ANCC)

• Valid California driver's license
  
• Minimum two years of Nurse Practitioner experience in one or more of the following:
