Job summary

QuantumCare Wound Care seeks dedicated Nurse Practitioners or Physician Assistants to join our mobile care team providing in-home and clinic visits. You will perform comprehensive health assessments, manage chronic and acute wounds, and deliver advanced wound care with a patient-centered approach.

Applicants must hold an active NP or PA license in Nebraska or Iowa, have wound care or geriatric care experience preferred, and be comfortable traveling to patient homes and facilities.

Qualifications

• Active and unrestricted NP or PA license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation with valid driver’s license for day-trip travel to homes/facilities.
• Strong clinical assessment, communication and documentation skills.

Responsibilities

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
• Deliver mobile wound care services directly in patients' homes or care facilities, ensuring comfort, accessibility, and continuity of care.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.

We are seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile care team.
