Carease Health seeks an experienced Nurse Practitioner or Physician Assistant with a geriatrics/internal medicine background to deliver preventive care to nursing home residents. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for documentation.

You will collaborate with care teams to develop individualized care plans and maintain accurate records, while contributing to quality initiatives to improve patient outcomes and facility ratings.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatrics.
• Experience with documentation review.
• Ability to work independently and remotely.

About the Role
