Job summary

One to One Health is seeking a PRN Provider to join our Indiana clinic team. You will evaluate and treat patients, manage acute and chronic conditions, and implement evidence-based treatment plans under the supervising physician.

The ideal candidate is a Board Certified Nurse Practitioner or Certified Physician Assistant with an active license, 1 year of family/internal/urgent care experience, and comfortable collaborating with senior leadership and the medical director. BLS and DEA preferred.

Qualifications

• Board Certified Nurse Practitioner or Certified Physician Assistant with an active license.
• 1 year experience in family medicine, internal medicine or urgent care setting.
• DEA license preferred.
• BLS certification desirable.

Responsibilities

• Perform pre-employment and annual wellness physicals.
• Manage acute and chronic health problems using evidence-based protocols under supervising physicians’ guidelines.
• Provide health screenings and disease prevention programs.
• Enter patient data into the EMR system accurately.
• Collaborate with multidisciplinary team and report to medical director.
