McGregor PACE is a Program of All-inclusive Care for the Elderly seeking an Adult/Geriatric Nurse Practitioner to join our Interdisciplinary team. The role centers on delivering comprehensive primary care in collaboration with on-site physicians for frail older adults in our Brooklyn and Elyria offices.

Qualifications

• Master's degree in nursing with FNP or AGNP certification and Ohio license.
• At least 2 years direct care with frail older adults.
• At least 2 years advanced practice nursing experience.
• Experience in interdisciplinary care settings preferred.
• Bilingual (English/Spanish) encouraged.
• CPR (BLS) certification required.

Responsibilities

• Provide primary care with on-site physician collaboration to a panel of participants.
• Manage acute and chronic illness, promote wellness, and follow patients across outpatient settings.
• Collaborate with IDT including nursing, social work, therapy, transportation, dietary and mental health services.
• Conduct ongoing assessments to create and implement comprehensive care plans.
• Serve as a clinical resource and preceptor for clinical orientation; may precept students.
• Assist in participant data collection and performance improvement initiatives.
• Work within Ohio and Federal scope of practice and licensure requirements.
• Maintain continuing education and prescriptive authority requirements.
• Participate in clinical on-call rotation (from home).

Job description

McGregor PACE is a Program of All-inclusive Care forthe Elderly (PACE), which provides wrap-around services that address the medical, functional, social, and personal healthcare needs of frail seniors living in our community.

We are seeking an Adult /Geriatric Nurse Practitioner to work in collaboration with our Interdisciplinary team in a supportive practice environment. The position requires an energetic, risk-tolerant applicant who enjoys working in a team designed to support elders at risk for nursing home placement- and help them remain in their homes

Hours are Monday through Friday, day shift only covering our Brooklyn and Elyria office locations (west side).

Essential Job Functions:

• Works as primary care provider with on-site primary care physician collaboration to deliver comprehensive health services to their panel of participants, including assessment, diagnosis, clinical decision making, treatment and prescribing.
• Directly manage acute and chronic illness, promote wellness and preventative care, and follow their patients into various outpatient care settings (including skilled and long-term nursing facilities, assisted living facilities, group homes, and participant homes) in collaboration with the Interdisciplinary Team (IDT).
• Interdisciplinary support includes on-site integrated collaboration with nursing, social work, physical/occupational/speech therapy, transportation, dietary services, and mental health services.
• Together with the IDT, provides on-going assessments with a focus on physical, mental and behavioral needs to create and implement a plan of comprehensive care.
• Serves as a clinical resource for nursing staff and preceptor for clinical orientation. Opportunities for precepting students (not required).
• Assists in program and participant outcome data collection and analysis; participates in performance improvement initiatives.
• Is responsible for identifying and working within the scope of practice permitted by Ohio and Federal laws and regulations, the professional code of ethics, and professional practice standards
• Responsible for achieving continuing education and other requirements for licensure, certification and prescriptive authority
• Participates in clinical on-call rotation (from home)

Qualifications:

• Masters prepared nurse with certification as a Family Nurse Practitioner or Adult/Gerontology Nurse Practitioner holding active, unrestricted license in the state of Ohio, required.
• Minimum of two years direct care experience working with frail, older adults in community, long term, skilled care, or hospital, required
• Minimum of two years advanced practice nursing experience, required
• Previous experience in interdisciplinary care setting, desirable
• Bilingual candidates (English/Spanish) are encouraged to apply
• CPR (BLS) certification, required
