Job summary

The Nurse Practitioner role with TEEMA in Wisconsin involves assessing, diagnosing, and managing healthcare needs of incarcerated individuals within a multidisciplinary team. You will provide evidence-based care, coordinate treatment plans, and ensure compliance with policies and standards.

Responsibilities include comprehensive patient evaluations, prescribing under state regulations, monitoring chronic diseases, preventive care, and collaboration with physicians and mental health professionals

Qualifications

• Current Nurse Practitioner license in Wisconsin (or ability to obtain prior to start).
• Current BLS certification.
• Minimum one 1-2 years of Nurse Practitioner experience.
• Preferred: experience in primary care, family practice, internal medicine, urgent care, emergency medicine, occupational health, or correctional healthcare.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating within a multidisciplinary team.
• Experience serving diverse and underserved populations.

• Diagnose and treat acute illnesses, injuries, and chronic medical conditions.
• Order, interpret, and evaluate laboratory and diagnostic testing.
• Prescribe medications and treatments in accordance with state regulations and clinical protocols.
• Develop and manage individualized treatment plans.
• Monitor and manage chronic diseases including diabetes, hypertension, asthma, COPD, and other ongoing conditions.
• Provide preventive healthcare, wellness counseling, and patient education.
• Respond to urgent medical situations and collaborate with onsite healthcare staff to ensure appropriate patient care.
• Maintain accurate, timely, and compliant medical documentation.
• Collaborate with physicians, nursing staff, mental health professionals, and other interdisciplinary team members.
• Participate in quality improvement and patient safety initiatives.
