Som Medical Practice is seeking a passionate Nurse Practitioner to join our growing team serving Niagara Falls, Olean, Lockport, and Hamburg. You will provide comprehensive primary care in a collaborative clinical environment and deliver high-quality patient care.

Responsibilities include performing exams, diagnosing illnesses, developing care plans, ordering tests, prescribing medications, and documenting in the EHR.

Qualificações

• Active NP license in New York state.
• Master's or Doctoral degree in Nursing from an accredited program.
• Current board certification (FNP-C or ANP-C preferred).
• Valid DEA license with NY prescriptive authority.
• Experience in primary care or outpatient settings preferred.
• Proficiency with electronic health record systems.

Responsabilidades

• Conduct comprehensive patient assessments including exams and histories.
• Diagnose and treat acute and chronic illnesses per guidelines.
• Develop and manage individualized care plans and protocols.
• Order, interpret, and follow up on labs and imaging.
• Prescribe medications and educate patients on management.
• Collaborate with physicians and care teams to ensure continuity.

Responsibilities:

• Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations
• Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines
• Develop and manage individualized patient care plans and treatment protocols
• Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies
• Prescribe medications and provide patient education on medication management
• Collaborate with physicians, specialists, and care team members to ensure continuity of care
• Maintain accurate and timely documentation in the electronic health record (EHR) system

Requirements:

• Active Nurse Practitioner (NP) license in the state of New York
• Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
• Current board certification (FNP-C, ANP-C, or relevant specialty certification preferred)
• Valid DEA license and prescriptive authority in New York State
• Experience in a primary care, internal medicine, or outpatient clinical setting preferred
• Proficiency with electronic health record (EHR) systems
• Strong interpersonal and communication skills with a patient-centered approach
