Somos Medical PC is actively seeking Licensed and Board-Certified Nurse Practitioners (NPs) to join our expanding network of community-based primary care practices across the Bronx, Brooklyn, Manhattan, and Queens. This is a full-time outpatient opportunity where you will play a vital role in providing comprehensive, patient-centered care to diverse populations in New York City.

Roles and Responsibilities:

Provide high-quality, evidence-based primary care to patients of all ages.
Conduct comprehensive health assessments, including medical history and physical exams.
Diagnose and manage acute and chronic conditions with a focus on prevention and wellness.
Educate patients about diagnostic tests, treatment plans, medications, and preventive care.
Prescribe medications and therapies in accordance with state regulations and best practices.
Identify and refer patients to specialists and ancillary healthcare services as needed.
Work collaboratively with physicians, nurses, and medical assistants to promote team-based care.
Support clinical operations and quality improvement initiatives to enhance patient care delivery.

Qualifications:

Must be Board Certified
BLS certification good to have
Excellent communication, bedside manner, strong ethic, and organizational skills.
Intermediate computer skills with experience with electronic health records.
Multilingual is a plus.
