Job summary

Hartford HealthCare Medical Group in Connecticut seeks a PH Nurse Navigator to support multidisciplinary care and coordinate care for pulmonary hypertension patients across inpatient and outpatient settings.

The nurse serves as a central liaison among patients, physicians, advanced practice providers, and care teams, ensures accreditation requirements are met, and helps optimize access to therapies while enhancing overall care quality.

Qualifications

• RN license and current licensure to practice nursing.
• Five years’ nursing experience OR one year experience in Cardio/Pulmonary Disease.
• Two years supervisory/management and patient education experience preferred.

Responsibilities

• Navigate patients through the continuum to remove barriers to care and improve satisfaction.
• Collaborate with the health care team and act independently to ensure prompt and efficient patient hand-offs.
• Apply the nursing process within the organization’s professional practice framework.
• Participate in community education outreach programs when needed.
