Job summary

Baptist Health in Miami is seeking a Nurse Navigator to coordinate care across the continuum, educate patients, and advocate for timely decisions and outcomes. The role requires an active RN license and a BSN is preferred, with oncology experience and EMR proficiency enhancing candidacy.

Qualifications

Minimum 3 years of clinical experience in a hospital, managed care environment or medical office.
Active RN license is required.
RN/BSN preferred; BSN highly preferred.
Oncology experience preferred; related certifications encouraged.
Knowledge of HEDIS and Star ratings, risk adjustment and HCC coding preferred for BHQN.
Proficient in computer applications and typing skills.

Responsibilities

Coordinate care across the continuum between patients, physicians and care teams.
Educate and provide information and support for patients and their families.
Facilitate decision making and advocacy for patients throughout the care journey.
Monitor patient outcomes and participate in quality improvement activities.
Collaborate with a multidisciplinary health care team to positively impact patient outcomes and experiences.
