Qualifications

• For Rural Health Designation clinics, BLS certification within 6 months of hire.

Responsibilities

• Provides direct patient nursing care for specified patients.
• Performs nursing assessments and triage of patient care needs for new and ongoing patients.
• Oversees planning, scheduling, and implementation of day-to-day clinical activities for navigated patients.
• Central point of contact for all navigated patients; coordinating with medical, nursing, and ancillary services.
• Provides education to patients, families, and significant others; acts as an information resource.
