Job description

Description

A professional RN with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an Oncology Nurse Navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.

Responsibilities
  • The ONN acts as an advocate to assist the high‑risk patient population in navigating internal & external systems to facilitate the diagnosis and treatment process. Participates in a new patient visit and education to help patients understand diagnosis, treatment options, and available resources.
  • Acts as a liaison between physicians, health care staff, and the patient to ensure a successful episode of care, following up after invasive procedures, surgeries, admissions, ED visits, and chemotherapy treatments.
  • Communicates and coordinates care among broader multidisciplinary teams and the patient’s additional provider teams, keeping them informed of the status of the patient’s care from the start of the new patient visit note.
  • Participates in quality‑metrics review to maximize outcomes, collects data, prepares reports for accreditation and site‑specific needs, and assists with quality‑improvement activities.
  • Coordinates patient paperwork needs, monitors and manages MediTech worklists, triages critical results, and monitors for new symptom‑management needs; assists with updating care plans and oncology history.
  • Assists in policy and procedure development, revision, and implementation to maintain compliance with state and national guidelines, standards, and coding regulations.
  • Participates in staff education on an individual or group basis; updates the nurse manager on changes that could impact care delivery; maintains regulatory knowledge of specialty area; engages in professional development and ongoing learning.
  • Completes any other duties required or delegated to the Oncology Nurse Navigator and assists with clinic throughput and efficiency as needed.

  • Attend and participate in at least 50% of department staff meetings; sign off on review of 100% of department staff meeting minutes.
  • Complete all required annual education by the due date.
Knowledge, Skills and Abilities
  • Minimal physical effort: occasional light lifting up to 25 pounds; stooping; picking, pinching, typing, or other finger‑based work.
  • Ability to manage considerable mental stress.
  • Ability to express or exchange ideas orally.
  • Ability to receive detailed information orally.
  • Ability to communicate effectively with co‑workers and convey information clearly, concisely, and accurately.
  • Familiarity with computers and other business machines.
Qualifications

Education: Nursing degree.

Experience: Prior experience in an oncology environment preferred; care coordination or navigation helpful. Minimum of two years of clinical specialty experience.

Licenses/Certifications: Current NH RN, BLS, and an ONCC‑related oncology certification obtained within 12 months of hire.
