Job summary

Silver Cross Hospital is seeking an Oncology Nurse Navigator to coordinate patient care through the cancer care continuum. The role acts as a clinical liaison, assessing needs and facilitating treatment planning with the multidisciplinary team.

The position requires an RN with an Illinois license and BSN, with two years in hematology/oncology; OCN or chemotherapy provider credentials preferred. This on-site role supports discharge planning, education, and quality initiatives within Cancer Center.

Qualifications

• RN with Illinois license; BSN required.
• Minimum two years hematology/oncology nursing experience.
• OCN or Oncology Chemotherapy Provider preferred.

Responsibilities

• Coordinate patient care across services and departments.
• Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs.
• Educate patients and families on disease, treatments, side effects, and reportable signs/symptoms.
• Coordinate discharge planning with family, social workers, home nursing, oncologists, and physicians.
• Document multidisciplinary recommendations and care plans.
• Promote education and preventive screening programs.

Acts as a clinical liaison for patients, across medical services and clinical departments. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs.
