Job summary

Silver Cross Hospital seeks an RN Navigator for Oncology to coordinate patient care across disease stages and act as a liaison among patients, families and the multidisciplinary team.

The role requires an Illinois RN license, BSN, and oncology experience; OCN/chemo provider certifications are preferred. Duties include education, discharge planning, and patient advocacy in a busy cancer center.

Qualifications

  • Registered Nurse with Illinois license and BSN degree.
  • Oncology Certified Nurse and Chemo provider certifications preferred.
  • Minimum two years hematology/oncology nursing experience with cancer care knowledge.

Responsibilities

  • Coordinate care for oncology patients across the disease course.
  • Act as clinical liaison for patients, families and the care team.
  • Assess clinical, psychosocial, and financial patient needs and educate patients and families.
  • Coordinate discharge planning with care team and families.
  • Document recommendations from multidisciplinary conferences and care plans.

Acts as a clinical liaison for patients, across medical services and clinical departments. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Evaluates and manages patient symptoms before and after treatment**.****Essential Duties and Responsibilities:*** Act as liaison for patients, families and their multidisciplinary clinical team.* Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs.* Facilitate resolution to patient needs and monitors follow up.* Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.* Coordinate discharge planning with the patient’s family, social workers, home nursing staff, oncologists, and personal physicians.* Work with patients and family members to ensure discharge instructions are understood and followed.* Develop and/or coordinate development of patient education materials, clinical pathways, and quality studies.* Coordinate and/or participate in multidisciplinary consultations and/or tumor conference.* Document recommendations made at multidisciplinary consultations and/or tumor conferences.* Help develop a treatment plan that is understandable and feasible to the patient.* Serve as clinical partner and escalation point for Intake Coordinators.* Promote community education and increases prevention awareness and early-detection screening programs.* May be responsible for direct patient care* Perform other duties assigned* Adhere to Silver Cross policies and professional standards.* Promote a clean and safe environment of care, utilizing the SAFE error prevention habits.* Provide the highest standard of privacy and confidentiality in matters involving patients, coworkers, and Silver Cross by abiding by the Standards of Conduct**Required Qualifications**:**Education and Training**:* Registered Nurse with a current Illinois license.* Graduate of an accredited School of Nursing with BSN.* Oncology Certified Nurse preferred.* Oncology Nursing Society Chemotherapy Provider preferred.* Minimum of two years hematology/oncology clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients with a cancer diagnosis, and knowledge about clinical aspects of oncology including protocol therapy and data management.* Must have general computer skills and be adaptable to changing working situations and work assignments.* Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.* Illinois nursing license required.* OCN Certification preferred.* ONS Chemotherapy Provider preferred.**Work Shift Details:**Days, M-F No holidays or weekends 8 hour shifts**Department:**CANCER CENTER NL# # # # # # # **Benefits for You**At Silver Cross Hospital, we care about your health and well-being and that is why we work hard to provide quality and affordable benefit options for you and your eligible family members.**Silver Cross Hospital and Silver Cross Medical Groups** offer a comprehensive benefit package available for Full-time and Part-time employees which includes:· Medical, Dental and Vision plans· Life Insurance· Flexible Spending Account· Other voluntary benefit plans· PTO and Sick time· 401(k) plan with a match· Wellness program· Tuition Reimbursement**Registry employees who meet eligibility may participate in our 401(k) Savings plan with a potential match.** However,registry employees are ineligible for Health and Welfare benefits.The expected pay for this position is listed below:$35.00 - $42.50The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.
