Qualificações

• BSN from an accredited program.
• Current Illinois RN license.
• AHA CPR certification.
• Strong English communication skills, both verbal and written.
• Experience within the past five years.
• Basic computer skills.
• Ability to use interpreter services when needed.
• Prior case management or patient navigation experience preferred.

Responsabilidades

• Collaborate with patient, family and healthcare team to coordinate care.
• Assess patient needs and coordinate multidisciplinary referrals and appointments.
• Triages information and schedules patients appropriately.
• Educate patients and caregivers about diagnosis, therapies and testing.
• Support transitions in care and care providers.
• Utilize evidence to guide practice decisions and participate in research when applicable.
