Job Summary:
The Nurse Navigator works closely with Physicians to coordinate a patient’s care. Under medical and administrative direction, they provide patient education related to their illness and planned treatments. The Nurse Navigator provides leadership and nursing services to patients in our community health setting and works with the health care team of medical providers to provide quality health care services for our patients. They work closely with the CMO, Providers, and Chief Operations Officer.

Wide range of standard medical equipment including oxygen tanks, wheelchairs, thermometers, etc. Computer use and proficiency required.
Work Environment:
Medical exam/treatment rooms and medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.
Mental/Physical Requirements:
Involves standing, walking, kneeling, bending, grasping, manipulating and squatting. Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings or make presentations at various sites when required. Stress can be triggered by workload and complex cases.
